You missed an appointment. Write a letter explaining what happened. In your letter include:
what the appointment was
a apology for missing the appointment
a request to reschedule
Dear Madam,
I like to apologize for missing my dental cleaning appointment at 11 am on Monday, 4th of July. Please allow me to explain the reasons for not showing up for my appointment.
As you remember, I have a 4 year old daughter named Sara, who just started attending preschool. On the day of the appointment, I dropped her off to her preschool and was en-route for my dental cleaning appointment. On my way, I got a call from one of the teachers from the preschool to tell me that Sara had just slipped during their play-time and had started bleeding from her nose. I immediately went back to the preschool to take her to the hospital so that she gets immediate medical attention. Due to all this, I was also unable to call your receptionist to cancel my appointment and try to reschedule it. I am extremely sorry to keep you waiting your precious time. But, I had to take care of my daughter and hence could not show up on my appointment time.
I hope you understand my situation as a fellow parent and forgive me. I would like to reschedule our appointment to another day whenever you are free during the next week.
Yours Sincerely,
Nitin
