Dear Mr. Jack,
I am writing to inform you about a serious health issue, which I am currently struggling with due to the working conditions at the workplace, and I would like to suggest ideas to improve the situation.
With regard to the health problem, I have been suffering from severe back pain for the last 15 days. In fact, this is extremely painful that I could not sit on my office chair and have consulted a doctor, who suggested taking two weeks rest and has prescribed some pain relief medicines to me.
As for the reason why it is due to the working conditions in the office, I could only assume it was the result of newly introduced office chair. Because this chair is not designed considering long working hours and quite uncomfortable from the body posture point of view.
Because all these chairs have health-related concerns, may I suggest you please contact the facility team and recommend that every employee should be allocated an ergonomic chair to work comfortably at the workplace.
