Respected Sir,
I am Mr Shreyas Parab, student of first year Bachelor's in Management course, the identification number is 123456. Due to unavoidable circumstances, I would request you to grant me a precautionary leave for seven days.

Recently, while working part-time in a restaurant, one of our colleagues has tested positive for COVID-19, and we have come in contact frequently in the daily chores assigned to us. So far I have no symptoms, additionally, the test result is negative in my case, however, as a preventive measure it has been asked by the healthcare authorities to isolate myself for consecutive seven days, before taking another test.

I request you to grant me the leave, also to extend the deadline for project submission which is due next week. Additionally, I request to provide provision to attend lectures online, until I get my another medical test result negative.

For your reference, the medical report is attached with the letter, and the letter hardcopy is sent through my friend as a safety protocol.
