Dear Sir/Madam,
I am writing to inform you that i cannot return the Physiology text-book i borrowed from the library on the due date 25th of August 2019. Regrettably, i have to travel to yorkshire to visit my cousin james who is critically ill.

james study at yorkshire university, yesterday i got an emergency phone call that he was seriously sick. The doctors said he has had mini stroke, also known as transient ischemic attack. As the only relative he have in this country, i have been told to come take care of him for a while. i will be spending two weeks in yorkshire and i will not come back in time to return the book i borrowed before its expiry date.

Since i cannot meet up the deadline for returning the book, i would like to renew the loan of the book for an extra two weeks.

i apologise for the inconvenience, looking forward to your prompt response.
