Dear Mrs. Aggarwal,

I am writing this letter to request for a reference letter in my favor. I am applying for a job in Johnson multispecialty hospitals. It is one of the best hospitals in town, I have applied for a senior physical therapist in the physiotherapy department, I have to do all the initial evaluations of the patient along with a proper plan of care, I am responsible for documentation as well.

It was my dream job. I always wanted to work in that hospital. They have asked me to submit a reference letter for my past experience. I request you to provide me with a reference letter as during my duration of the course, I am been performing similar responsibilities. This letter is really important as it will highlight all the responsibilities which this job profile requires.

You can write about the total experience along with the duties I have performed during my course and how I was at doing my job.
I will really appreciate it.
