Dear Sir/Madam
I am writing to inform you of the issues faced by me after I lost my student identification card. Recently I have shifted my accommodation to Smith's street which is not far away from the college, and I am sure that I had put it in the bag i usually carry along with me, but I fear about throwing it in trash with other waste papers while I was making some room for putting the documents in the already untidy table drawer.
As a result of this incident, various facilities such as medical care or transport, costs me very dearly. Every month I receive fixed amount of salary from the part-time job and the pocket-money, however past few days have turned out really hard to manage the expenses, and eventually I could end up with no finances for the survival if I do not get the ID proof soon.
Therefore, please accept my sincere apologies on losing it, and I urge you to allot a new card as soon as possible. I assure you that I will be more cautious in future.
